It’s surprisingly easy. All the other nonsense on the web is out of date.
Restart your Quicktime Player app and restart your iPhone.
Plug your iPhone into your Mac.
Choose New Movie Recording from Quicktime Player.
(Don’t choose New Screen Recording.)
Then choose the dropdown next to the little red button.
Then click the red button and you are recording!
I had a hell of a time getting it to show up so I restarted everything. Only works on OS X Yosemite or later and with Lightning connector (i.e. recent iPads/iPhones)
